NATIONWIDE MEDICAL PLANS – CIGNA
Each medical plan offered through Cigna utilizes the Cigna Open Access Plus (OAP) network. You can use any doctors or facilities, but you will save money by receiving services with an in-network provider.
Plan Highlights:

- No referral needed for In-Network or Out-of-Network specialists
- Pre-certification may be needed for hospitalizations and some types of care
- Triple tax advantage with an HSA – requires enrollment in Cigna Choice Fund HSA medical plan

REGIONAL MEDICAL PLANS – KAISER
If you reside in a service area zip code in California, Colorado, or Washington, you have access to the Kaiser Health Maintenance Organization (HMO). You will need to choose a primary care physician (PCP) from their network of providers to coordinate your care and receive referrals to specialists. You pay a copayment for some products and services, and there is no annual deductible.

- No deductibles
- Most services covered 100% after copay
- No claim forms required – unless for emergency services.
HAWAII MEDICAL PLANS – HAWAII MEDICAL SERVICE ASSOCIATION (HMSA)
If you reside in Hawaii, you have access to the HMSA medical plans. Headquartered on Oahu, HMSA is an independent licensee of the Blue Cross and Blue Shield Association. Depending which plan you choose, you may need to choose a primary care physician (PCP) to coordinate your care. However, both plans provide access to quality care and resources.

- No deductible.
- Most services covered 100% after copay.
PPO Highlights:
- Large network of providers.
DENTAL
MAINLAND DENTAL PLAN – METLIFE
Dental care coverage is available through MetLife. This plan allows you to receive care from any dentist you choose. However, when you obtain care from one of MetLife’s PDP Plus network dentists, your Out-of-Pocket expenses are reduced. To locate a network provider, search the Provider Directory at www.metlife.com, click on Employee Benefits, and select the PDP Plus network.
Plan Highlights:
- Large network of providers
- Deductible waived for In-Network x-rays, routine exams, and teeth cleanings
- 90% coverage for basic services; 60% coverage for major restorative after deductible
- Orthodontia available to children up to age 21
- Available in all states except for HI

HAWAII DENTAL PLANS – HAWAII MEDICAL SERVICE ASSOCIATION (HMSA)
If you enroll in an HMSA medical plan, you have a choice of dental plans. You can select from either the HMSA Dental Network Program (HMO) or the HMSA Participating Provider Dental Program (PPO). Please note that for the HMSA HMO Plan, all services must be provided or coordinated by an HMSA Dental Network Provider. For the HMSA PPO Plan, you may use any licensed dentist you wish; however, using dentists who participate in the HMSA Dental Provider Network will save you money and offer greater benefits.

VISION
MAINLAND VISION PLAN
Vision care coverage is available through VSP for mainland employees. VSP allows you to receive vision care services from any provider you wish; however, most eligible services received by In-Network providers are fully covered after your copayment. Vision care received from out-of-network providers is reimbursed to the patient up to the applicable maximums.

EMPLOYEE ASSISTANCE PROGRAM
EMPLOYEE ASSISTANCE PROGRAM – CONCERN
Swinerton has partnered with CONCERN to offer our employee assistance program, also known as EAP. This free and confidential program helps all employees and their families balance the demands of work, life and personal issues. The program includes 24/7 telephone access to licensed behavioral health professionals and up to ten face-to-face visits with a counselor per incident each year.
Plan Highlights:
- Up to 10 counseling sessions per issue per year.
- Assistance available 24 hours/day, 7 days/week.
